Help using online Referral Form<\/h2>\n

To activate the online referral form, click the link (you will need at least Adobe Acrobat Reader 7). Acrobat Reader 7 is free software which may be downloaded from\u00a0Adobe<\/a>.<\/p>\n

The form may be completed online and submitted by email, or printed off and completed by hand offline.<\/p>\n

To complete and submit ONLINE:<\/h4>\n
    \n
  1. Open form by clicking on the link.<\/li>\n
  2. Complete all known patient information by clicking on check boxes or typing into form.<\/li>\n
  3. Check all information is correct.<\/li>\n
  4. If you wish to save a copy for your files, select “Save As” from the “File” menu.<\/li>\n
  5. Once ready to send referral form, click the “Submit” button. A new email message will open in your default email service (eg Outlook) with the form as an attachment.<\/li>\n
  6. Add a message in the body of the email, including your name and contact details.<\/li>\n
  7. Send email.<\/li>\n
  8. To send the referral form using a web-based email server (eg hotmail, yahoo) you will firstly need to save the referral form by selecting “Save As” from the “File” menu.<\/li>\n
  9. Open your web-based email account and create a new message.<\/li>\n
  10. Attach the completed referral form to the email.<\/li>\n
  11. Include your name and contact details in the body of the email and send to:\u00a0info@anzrag.com<\/a>.<\/li>\n<\/ol>\n

    To complete and submit OFFLINE:<\/h4>\n
      \n
    1. Open the referral form by clicking the link.<\/li>\n
    2. From the “File” menu, select the “Print” option.<\/li>\n
    3. Print the blank form to complete by hand.<\/li>\n
    4. Submit the referral by fax to: 08 8404 2035 or through the mail to:\n
        \n

      • Dept of Ophthalmology, Flinders Medical Centre<\/li>\n
      • Flinders Dve<\/li>\n
      • Bedford Park SA 5042.<\/li>\n<\/ul>\n<\/li>\n

        TROUBLESHOOTING:<\/h4>\n
          \n
        1. Some browsers will open the form in their own viewer rather than Adobe Acrobat Reader.<\/li>\n
        2. Option 1: Right click the Referral Form link and save to desktop.<\/li>\n
        3. Open form from your desktop, complete and click Submit.<\/li>\n
        4. Option 2: Change the PDF settings in your browser<\/li>\n
        5. In Chrome: Click the 3 dots at the top right corner.<\/li>\n
        6. Settings> Advanced>Privacy & Security>Content<\/li>\n
        7. Scroll to bottom and find PDF. Select Open Using Different Application<\/li>\n
